Psychiatric Evaluation
New Patients · 60 min

Your first appointment is a comprehensive, unhurried evaluation. Dr. Miceli will review your full psychiatric history, current and past medications, prior diagnoses, relevant medical history, and — most importantly — your own understanding of what has and hasn't worked.

At the end of the evaluation, you will receive Dr. Miceli's clinical impressions, a preliminary diagnostic formulation, and a proposed treatment plan. You will have time to ask questions and to participate actively in decisions about next steps.

Medication Management

Follow-up appointments focus on monitoring your response to treatment, adjusting medications as needed, reviewing side effects and tolerability, and ensuring your treatment plan continues to reflect your current needs and goals.

Dr. Miceli's philosophy is to use the minimum effective treatment — adding complexity only when clinically necessary, and regularly reassessing whether existing medications remain appropriate.

Deprescribing Consultations

Deprescribing consultations are available to patients who are seeking a structured, evidence-based review of their current psychiatric medications — with the goal of safely reducing or eliminating medications that may no longer be necessary, effective, or appropriate.

These consultations are comprehensive. Dr. Miceli will review your complete medication history, the original indications for each medication, how your clinical picture has evolved over time, and what an individualized tapering plan might look like for you.

Deprescribing is not appropriate for everyone, and it is never rushed. See the section below for a full explanation of the process, its benefits, and its risks.

Telehealth Psychiatry
New York State Only

All services offered at this practice — including initial evaluations, follow-up visits, and deprescribing consultations — are available via secure, HIPAA-compliant video visits for patients located in New York State at the time of the appointment.

Telehealth visits are billed at the same rate as in-person visits. Patients are responsible for ensuring a private, stable connection and for notifying the practice if they will be located outside of New York State at the time of a scheduled telehealth appointment.

What deprescribing is — and what it isn't.

Deprescribing is the systematic, clinically supervised process of reducing or discontinuing psychiatric medications that are no longer necessary, no longer effective, or causing more harm than benefit.

It is not simply stopping medications. Abrupt discontinuation of many psychiatric medications can cause serious withdrawal symptoms or rapid return of the underlying condition. Responsible deprescribing involves a gradual, carefully monitored taper — often slower than patients expect — with close attention to any emerging symptoms.

Not every patient is a candidate for deprescribing. Some patients benefit significantly from long-term psychiatric medication, and that is a valid clinical outcome. The goal of a deprescribing consultation is honest assessment: which medications are still helping, which may be safely reduced, and what a safe path forward looks like for this specific patient.

Risks to understand before you begin.

Withdrawal
Some medications cause discontinuation symptoms even when tapered gradually. These can include physical and psychological symptoms.
Symptom Return
Underlying conditions may re-emerge as medications are reduced. This is monitored closely and addressed clinically.
Variable Timeline
Some patients require very slow, prolonged tapers lasting months to years. The pace is determined by your response.
Unpredictability
Individual responses to tapering vary. Close communication and regular monitoring are essential throughout the process.
